He was hit head on by a truck after he (Zanardi)  veered into the oncoming lane.  Multiple injuries, very serious condition. Breathing when he was placed on the helicopter. Sounds bad.

Its pretty common even in the US to only close one direction for bike races.

Sometimes if it is a divided street with multiple lanes, only one lane is closed. Sometimes no lanes are closed. I used to hate when my triathlete son raced and the bike course included active roads with cops only at the entrance/exit to the course from the closed street/parking lot where the bike course started.
