The cars the Pope uses are typically built by the manufacturers or different business associations or other groups and given as gifts. Generally, someone gets the idea to do a car for him as a publicity stunt or something and approaches the Vatican to see if they’d be willing to accept it and get any crucial specs that must be incorporated for it to be usable, and they run with it from there.
