Note the ladders leading down into the crankcase.

What amazes me must about these big engines is their slow speed and minimal vibration. 22 to 102 rpm

I laughed my head off at that!
102 RPM!!!!

Is this from an article or video? I needs to see more!
I saw one once in an episode of Toughest Fixes or something, holy cow. But they mentioned nothing of the rpms/speed/vibrations

Needs to be a flat plane crank.

Just got off a cruise ship with several of the slightly smaller versions of these and I went all over the ship trying to find a place you could really hear and feal the engines and you couldn't. They are amazing. .. super dirty, but amazing

Want some other mind bending numbers? The biggest of these puts out 102000 hp but hp =torque x rpm/5252 = wait for it....5.25 million ft lbs of torque

During the climax of the movie, this is where the main character's arch nemesis will be ground to death.

Whatever it wants.
Also container ships.
Mostly the latter.

In college, I had a job at a cogeneration plant. I was night shift operator on four of these engines. Not as big as the one in your picture, but see the connecting rod in the foreground for scale.

Sort of. not that big but you wouldn't want your hand near a locomotive engine.
