Long story short, person used the brakes a lot. Person never bothered to have them maintained. One day, they made a loud bang, and the car didn't stop like it once did. The end.
Basically they ran the rotors down so far, the last bit of metal separated from the vents in the middle. In a lot of the /r/JRITS images, this doesn't happen, and it just starts to mash down on the vents...This is more impressive.

What the FUCK!? It looks like some sort of medieval torture device...I didn't even know this could happen. I can't believe someone could actually wear the brakes down this far, and by can't I mean I borderline refuse to .
I mean shit, I would've figured with sparks flying from metal on metal at at least both of the front brakes, and maybe even the rears, the car would've caught fire and burned to a crisp before the rotors could get this bad.

This happens surprisingly often. I have seen halved disc in here too even though the brake discs are yearly checked during the mandatory inspection. Usually it happens when one of the pads doesn't anymore have any friction material bud driver doesn't care about new noises.
Weirdest disc failure that I have seen was in my friends newly acquired Audi 200. He thought the brakes seemed a bit unbalanced. He checked one corner and noticed that the disc's friction surface looked very rusty and didn't have wear marks from the pads. Then he noticed that the bell had separated from the disc part and the disc part was just stuck and hanging in the caliper. Then he picked up the MIG welder and fixed the problem. I was quite horrified.
