Plain paper, Yes. Freehand, no, I used a stencil. But you could do it freehand with this method

Yes, mostly. The best Advice I ever got on drawing was from my observational drawing teacher: "Dont draw what you see, Draw your own interpretation of what you see" Its not about being accurate to the object, its about being accurate to your perception of the object. That can mean many things, but it stuck with me.
