It is a step. Most likely for accessing loads that went over the cab, above the rear rack. You can see this truck once had running boards, the rear support is visible below the rear door. The fender insert step is most likely an aftermarket accessory.

It’s for the placement of the left foot of an exotic dancer...who has clearly fallen off some time ago and likely took out the aerial as they did. If you look carefully you can see the ringbolts for the dancer’s retaining wires at the leading edge of the hood...

that circle behind the rear door is the fuel-filler hole...just missing a cap.
