Smallest car ever with a V12: 1960's Honda F1 car.
Smallest PRODUCTION car with a V12? Likely a short wheelbase Ferrari 250. That’s stretching the term ‘production’ so if not that then the Miura. Those things are tiny.

For a road legal car, I would have to say the Ferrari 250 GT California Spyder SWB. Pretty damn small
characteristic dimensions: outside length: 4410 mm / 173.6 in, wheelbase: 2400 mm / 94.5 in
Compare that to a ‘65 Shelby Cobra
characteristic dimensions: outside length: 3962 mm / 156 in, wheelbase: 2286 mm / 90 in
They are not too different in size, just price and quantity made (including replicas)

I answered earlier with the 166S, but the 125 S is smaller.
Ferrari 125 S : 1.5l V12, 1433 lbs.
I think it would be tough to find anything smaller (car or displacement) than that.

60's F1 cars were even lighter and smaller. For an example 1965 season’s Honda RA272 had 1.5l V12 and it weighed ~485 kg. An F1 is a car too...
