Here’s how I interpret this scene: Copious amounts of smoke/steam are pouring from the engine bay. They have obstructed the view of the road, allowing the car to miss a turn and drive of a cliff. As the car starts to plummet the smoke parts and the occupants see their impending deaths, and react with the expected terror.
