We usually don't have shopping carts. Unless there is a Target, Sears, Kohls or similar type big box store attached to the mall. The shopping carts aren't supposed to leave those stores, but some assholes still wander the mall with their shopping cart full of their purchases and one or more of the kids they can't handle.
But we do have those cart escalators in other stores that use carts and are more than one level.

Inside individual stores around here, but you can't take the cart out into the mall.

Luckily, no. People here aren't given shopping carts to begin with. Supermarkets are already a clusterfuck, I really don't want to know what it would be if we were allowed to use them mall-wide.

Only place I've seen a shopping cart escalator is the IKEA in Schaumberg, Illinois.

Ikea's carts have groved wheels so they won't roll on a regular escalator. Pretty simple solution.

Malls? No
Large multilevel stores do though. Off the top of my head the only one I can think of is the WalMart in Victoria, there's one (two?) going from the main level up to housewares and shit on the second floor... and then there's another going from the main floor to the underground parking

NOT universal in Canada. I see the cart escalators at one of the local "Canadian Tire" stores (kinda like "Pep Boys" meets "Pro/Ace Hardware" for non Canucks) but nowhere else.
Most malls don't allow or provide shopping carts; it's store specific.
At the local grocery store, they have escalators that don't have stairs; they're just long, gently sloping ramps with narrow grooves that lock into corresponding slots on the cart wheels, like this:

Malls? No. For the most part, malls in the US don't have shopping carts, but there's a multi-level Target store in downtown Seattle that has the cart escalator.

Malls here very rarely have shopping carts. But, I have seen those escalators in 2-story Targets and Walmarts.

Yep - some grocery stores or department stores that are on the 2nd floor have these.
