This came up recently in some comments; here’s a nice official Mazda video promoting the ND Miata, with this technique at 0:18
Edit: looking more carefully, it actually seems like a hybrid between “true” heel-toe and side of the foot. The Miata’s pedals look better suited to the heel than most cars seem to.

This is the method I use. With large feet (12 4E) and small footwells, it’s the only way.

I tend to do more of the side of the foot. Although with my semi-recent change to stainless brake lines and bleeding the system, the stiff pedal makes it a bit trickier than it was

Exact same thing here; size 13D. None of the sporty cars I’ve driven for any length of time (RSX, WRX, Miata, RX7, Mustang) have allowed me to do it any other way.
I’ve seen videos of people actually doing it with their heel and toe, but I genuinely wonder how small your feet would have to be to do it that way.

I was struggling to heel-toe with my actual heel, and switched to this side-to-side method. I think it’s a matter of personal preference/anatomy, I’ve seen videos of racing drivers doing side-to-side.

I don’t think I have ever driven a car with pedals far enough apart to heel&toe. It’s all been heel&sideofthefoot for me.

I’ve always been a side-of-the-foot type. I’ve tried using my actual heel and toe/ball of my feet and it’s just really uncomfortable and makes shifts difficult.

That’s the way I downshift in my SVT Focus. Pedals are placed perfectly for side-of-the-foot throttle blips. Never even thought of it anything but heel-and-toe, but if you’re going to be pedantic about it, it isn’t true heel and toe.
Twisting my leg far enough to use my heel on the pedal would be very uncomfortable.

Call it what you want, hitting two pedals with one foot is the entire point. Some cars have close pedals, some don’t.
