Has anyone ever made a dual cam-in-block engine? I'm thinking straight 6 OHV, but with an exhaust cam on the right and an intake cam on the left.

"New" Harleys are twin cam-in-block. Some large diesels may do the same, but my memory is foggy before my second cup of coffee.

Seems like an arrangement that'd serve OHV engines less well than a T-head. In fact, Wikipedia backs me up that that's what they did on T-heads. Up until the 50s, even.

Viper engine uses cam-in-cam phasing...wish it was available from GM, but I guess they don't want to license it. GM DID design the UV8 (ultimate v8) project that was incredible, but more costly than required to hit emissions/power targets at this time. I hope they revisit that eventually.
