Do you keep right unless passing on limited access, multi-lane roads? If not, why not?
"Because nobody passes me" is not a valid answer...
My only "why not" condition is when there is enough traffic that it no longer matters what lane you are in - but what do you do when there is a long break in the right lane?

This. Although it applies to all lanes not just the left most. People are pretty good about leaving the leftmost lane clear but always drive in the middle lane when right lane is open.

Its a law in my state but ~100% of people ignore it and its not enforced. This is a good resource for keep right laws

It especially bugs me when there is a crapload of traffic in the middle of 3 lanes and nobody in the right.

Its legalese for my state, but its actually super reasonably worded when you get past it.:
are you in the left lane? is there a car 2 seconds or less behind you? can you get out of the left lane? if yes to all and you don't move you are breaking the law.

My steering wheel is on the right. Therefore, the slow lane is the left lane. Keep left unless overtaking is the rule down here.
