I think captions are automatically unapproved, because before you could post a picture in an unapproved caption comment, and it would still show up when people click it. Rather than block unapproved ones, they made all captions unapproved for some reason regardless of authorship.

Happens to me every time I add an annotation, but it isn't REALLY unapproved.

This is a bug that we're working on fixing - however, even though it says it's in pending, it's actually not - so you should be good in that regard.
