He was most likely referencing the time Richard Hammond had a huge accident resulting in serious head injuries - which he recovered from.

I think he's trying to be positive and make light of the situation a bit. He followed up the tweet saying that what he meant by that was that even after serious brain injury you can return to exactly how you were before, he's poking fun at Hammond and using it as a reason to stay hopeful
