That doesn't count as an exception. It should read, "There are 200 McDonald's restaurants within 3 miles." McDonald's is a possessive determiner, which means it becomes an adjective in this case. Without the word "restaurants" there is no noun for the adjective to modify, hence we don't know exactly what there are 200 of. Unless you are at a reunion of the McDonald clan, and there are 200 McDonalds (plural, non-possessive) within 3 miles. Then the sentence works.
