Poor B-52 had some trouble:

Though I’ll admit it seems unlikely you could have just one engine fall off on a B-52, wouldn’t you lose a whole pod? They might have had to make do with only six engines.

Not really. The engines on the 52 are side-hung, with the pylon being a common mounting plate between them. It’s totally possible to lose one due to hardware failure and have the other stay attached just fine.

Somebody on another forum I frequent with some background (ex AF crew) was conjecturing that the maint door for that engine might not have been secured and might have suddenly blown open KABANG while at speed. It was the only thing for a single engine loss she could think of.
